New Patients

Provide us with medical records and immunizations within 1 week prior to your appointment. Please download our office forms and bring them with you to the office on your first visit.

School & Camp Forms

Please allow a minimum of 5 business days for completion of forms. Please keep in mind that the demand for forms increases during May, June and September so we ask that you allow additional time during those months. It is an office policy to charge for each form.

 

 

 

 

Insurance and Billing

We accept most insurance plans. For up to-date information, or further questions, please contact the office. We also encourage parents to be familiar with their insurance. This means being knowledgeable about co-payments, deductibles, and coinsurance as these are the patient’s responsibility and are regulated by the insurance provider. We also want to kindly remind parents that co-payments and all other payments are due at the time of service, unless other arrangements have been made in advance.

Referrals

If your insurance requires a referral, please allow ample time to have that referral available. We ask parents to have all the necessary information required to process referrals. Having all of this information handy will expedite the referral process.

After Hours/ Weekends: For an urgent matter that cannot wait until office hours, a physician will be on call. If you call the office phone number, you will be connected to 1 of the doctors through our answering service. If you do not hear back within one hour, please contact the answering service again.
